Registration for Placenta Services

Please fill out the contact form below. Shortly, you will receive a confirmation email that includes an attachment of my contract and guidelines for the process, including what you will need to do in the process. Please be sure to read the contract and client responsibilities- it should clarify the process greatly. You will also receive confirmation via text message. You can sign and return the contract to me via email, or in person upon pick-up of your placenta. Please note that full payment is required at pickup as well. All services and pricing are included on the main placenta services page.
*** Please note: for registration to be complete, $25.00 registration fee will be due. If you request multiple services, the registration fee will only be required once.
